Care Coordination & Client Advocacy
We work with families who have a loved one living in a nursing home, assisted living community, or private residence and need an experienced, independent set of eyes to help ensure their loved one is safe, supported, and treated with dignity.

Important note: Caledonia Dreamin does not provide medical or nursing care. We are not LPNs or medical aides. We work alongside medical professionals, families, and facilities to coordinate care, support decision-making, and advocate for client needs.

For clients who need broader support, we assist with the practical details that keep life running smoothly, including:

Professional
Fees
Caledonia Dreamin LLC provides individualized, non-clinical care coordination and advocacy services tailored to each client’s needs. Engagements are structured to ensure flexibility, discretion, and continuity of care.

Principal Rate: $75 per hour
​
Services are billed based on actual time worked and may include on-site visits, coordination, advocacy, planning, and communication with families and service providers.
​
Travel & Service Area
​
Services within a standard local service area are included in the hourly rate. For locations outside this area, travel time is billed at a reduced rate, along with applicable mileage. Travel arrangements are outlined during the initial consultation.
We begin with a thoughtful consultation to understand your situation and determine the most appropriate next steps.
