At Caledonia Dreamin LLC, we support individuals and families who need a reliable, organized advocate by their side. We specialize in care coordination, oversight, and life management for people navigating aging, complex family dynamics, and major life transitions.

Care Coordination & Client Advocacy 

We work with families who have a loved one living in a nursing home, assisted living community, or private residence and need an experienced, independent set of eyes to help ensure their loved one is safe, supported, and treated with dignity.

We provide:

  • Regular check-ins and on-site visits

  • Communication with facility staff and service providers

  • Oversight to help identify concerns early

  • Advocacy when families cannot be present or need backup

  • Organization of care plans, appointments, and next steps

Our presence helps reduce stress for families and provides peace of mind that someone is paying attention, asking questions, and following through.

Important note: Caledonia Dreamin does not provide medical or nursing care. We are not LPNs or medical aides. We work alongside medical professionals, families, and facilities to coordinate care, support decision-making, and advocate for client needs.

Life Management & Fiduciary Support

For clients who need broader support, we assist with the practical details that keep life running smoothly, including:

  • Coordination with trustees, wealth managers, and attorneys

  • Support for trust beneficiaries and individuals requiring oversight

  • Assistance with housing transitions and daily logistics

  • Mail, document, and property oversight for local and remote clients

Our work brings structure, accountability, and continuity—especially during times of transition or vulnerability.

Medical & Benefits Coordination (Non-Clinical)

We help clients and families navigate complex systems by organizing and coordinating:

  • Appointment scheduling and attendance (as an advocate, not a provider)

  • Communication between providers, facilities, and family members

  • Insurance, Medicare, Medicaid, and SSI processes

  • Referrals to appropriate licensed professionals and services

We do not diagnose or provide medical treatment.
We coordinate, document, and advocate—so nothing gets missed.

Housing Transitions & Daily Living Support

Whether someone is downsizing, relocating, or transitioning into assisted living, we manage the details that often overwhelm families:

  • Planning and coordination of moves

  • Making new living spaces safe and functional

  • Arranging services and ongoing support

  • Acting as a consistent point of contact during transitions

Professional Fees

Caledonia Dreamin LLC provides individualized, non-clinical care coordination and advocacy services tailored to each client’s needs. Engagements are structured to allow flexibility, discretion, and continuity of care.

​

Principal Rate — $75 per hour

Services are billed based on actual time worked and may include on-site visits, coordination, advocacy, planning, and communication with families and service providers.

Reduced rates may be available for first responders and public service professionals. Please inquire.

Care Coordination

Managing health, housing, and daily living needs can be overwhelming—for individuals, families, and trustees alike. Caledonia Dreamin LLC provides non-clinical care coordination and fiduciary support focused on stability, advocacy, and peace of mind.

We assist with:

  • Care Coordination & Advocacy — Scheduling, reminders, and support during medical, therapy, and wellness appointments.

  • Daily Life & Housing Support — Coordinating services, organizing personal tasks, and supporting safe living environments.

  • Financial & Administrative Organization — Assisting trust beneficiaries with bill management, budgeting, and communication with trustees.

Whether appointed as a guardian or working alongside families and fiduciaries, our role is to ensure continuity, oversight, and thoughtful follow-through.

Social Media &
Communications Strategy

In addition to client advocacy services, Caledonia Dreamin LLC provides strategic communications support for municipalities, public leaders, and campaigns. This work is handled separately from care coordination services and reflects experience in public trust, messaging, and community engagement.

Services include:

  • Campaign Communications & Strategy — Message development, content planning, and coordination.

  • Municipal & Public Outreach — Managing social media and public-facing communications.

  • Community Engagement — Strengthening public confidence through clear, consistent messaging.

Travel Planning &
Specialized Projects

For select clients, we also provide assistance with special projects, including thoughtfully planned solo travel and logistical coordination.

Support may include:

  • Custom itineraries and planning

  • Travel logistics and preparation

  • Ongoing coordination before, during, and after travel

 

These services are offered on a limited basis and tailored to individual needs.
